Patient Service Coordinator II

As a Patient Customer Service Associate II, you'll play a vital role in supporting both patients and clinical teams. You’ll be the connection between members, their healthcare providers, and essential services—helping ensure that care is timely, seamless, and compassionate.

What You’ll Do:
  • Act as a liaison between patients, healthcare providers, and service agencies to coordinate care and eliminate barriers to treatment.

  • Support patients in navigating their care plans by providing reminders, offering coaching for self-care, and helping them understand their health needs.

  • Collaborate closely with medical teams to gather essential information and help guide care decisions that lead to better health outcomes.

  • Assist with screening for changes in health status and implementing approved care interventions.

  • Manage the clinical referral and authorization process, ensuring that patients receive timely approval for medications, services, and specialty visits.

  • Handle administrative duties such as appointment scheduling, organizing medical records, drafting patient communications, and maintaining up-to-date files.

What You Bring:
  • Experience: Previous experience in healthcare or health plan environments required—especially with data entry, processing authorization requests, and working within electronic medical records (EMR).

  • Technical Skills: Ability to type at least 60 words per minute. Strong organizational and multitasking skills are essential.

  • Education: High school diploma or equivalent required.
